Brief
This contract involves the supply of a multi-channel fiber optic system for dynamic measurement of deformation and engineering structures as part of the project titled 'Public Safety Technology Center'. The supplier is responsible for delivering the equipment, its installation, commissioning, and providing user training at the designated location by the buyer, Wrocław University of Technology. The contract includes the provision of software, which entails delivering software media and transferring intellectual property rights to the buyer, allowing them to use the software as per the contract terms. All costs related to the execution of the contract, including delivery, insurance, risk, commissioning, and training, are to be borne by the supplier. The delivery location is Wrocław University of Technology, ul. Janiszewskiego 7/9, 50-370 Wrocław, building C-5, room 207. The detailed description of the contract subject is available in Annex 2 of the SWZ - OPZ. The detailed scope of work is outlined in the draft contract, which is Annex 3 to the SWZ. The required completion time for the contract is up to 90 days from the date of contract signing. The buyer reserves the right to cancel the procedure if the funds intended for financing the contract are not granted. The buyer will apply the so-called reverse procedure, where the evaluation of offers is conducted first, followed by the qualification of the highest-rated supplier. The supplier's offer is binding from the submission deadline until 03/08/2025 (90 days). The offer must include: 1. The supplier's offer form - an interactive form available on the e-Procurement Platform, indicating the total offer price. 2. A declaration as mentioned in section VII point 1 of the SWZ - JEDZ - an interactive form available on the e-Procurement Platform. 3. A declaration as mentioned in section VII point 1a of the SWZ (corresponding to declarations annexes 5.1 and 5.2 to the SWZ). 4. If applicable, a commitment from the entity providing resources as mentioned in section VII point 3.3 of the SWZ. 5. If applicable, a statement regarding the confidentiality of business secrets as per section X point 6. 6. Documents proving the right to sign the offer as per section X point 15 of the SWZ; if applicable, the relevant power of attorney as per section X point 16 of the SWZ. 7. If applicable, a statement from suppliers jointly applying for the contract as per article 117 paragraph 4 of the uPzp. 8. The required evidence - a description of the offered contract subject - technical specifications of the offered equipment along with any information for suppliers, allowing the buyer to verify compliance with the requirements presented in Annex 2 to the SWZ - completed Annex 2 to the SWZ. The buyer will consider the parameters indicated by the supplier in Annex 2 to the SWZ OPZ as evidence of compliance with the requirements. The requirement to indicate 'Name, type, model, manufacturer of the offered device' does not constitute evidence. According to article 107 of the uPzp, if the supplier does not submit the required evidence or if the submitted evidence is incomplete, the buyer will request their submission or completion within a specified timeframe. Before selecting the most advantageous offer, the buyer will request the highest-rated supplier to submit updated evidence within a specified timeframe, not shorter than 10 days, including: 1. A declaration from the supplier regarding article 108 paragraph 1 point 5 of the uPzp. 2. Information from the National Criminal Register regarding article 108 paragraph 1 points 1, 2, and 4 of the uPzp. 3. An excerpt or information from the National Court Register or the Central Register and Information on Economic Activity regarding article 109 paragraph 1 point 4 of the uPzp. 4. A declaration from the supplier about the accuracy of the information contained in the declaration. 5. A list of completed deliveries along with evidence confirming whether these deliveries have been duly executed or are being executed. 6. INFORMATION ON ELECTRONIC COMMUNICATION MEANS through which the buyer will communicate with suppliers, and information on technical and organizational requirements for preparing, sending, and receiving electronic correspondence - section IX of the SWZ.
