Supply of Pharmaceutical Products to Pro-Medica Hospital Pharmacy

This contract involves the supply of medications and pharmaceutical products to the Hospital Pharmacy of Pro-Medica in Ełk, Poland. The detailed specifications are outlined in the Pricing and Assortment Form, which is an attachment to the procurement documents. Additionally, the contract includes the leasing of medical devices, such as: 1) 1 vaporizer for the entire contract duration – Package 20 2) 4 vaporizers for the entire contract duration – Package 21 3) 52 enteral feeding pumps – Package 53. Partial bids are allowed (Packages 1-65). The contractor's responsibilities and the execution method are detailed in Attachment No. 5 to the procurement documents. Bidders must provide the commercial name, dosage, packaging size, quantity, material unit, manufacturer name, and EAN code for the offered products, especially for reimbursed medications. Offers will be rejected if the EAN code for reimbursed medications is not provided. If the offered medication packaging contains a different number of units, the contractor must recalculate the number of packages and round up to the nearest full package if necessary. The contracting authority may allow conversions based on a previously submitted inquiry. For medications with official prices according to the current announcement from the Minister of Health regarding the list of reimbursed medications, the contractor must include the EAN code in the pricing table. Specific packages have additional requirements regarding the source of the medications and their compliance with the current regulations.
