    Brief

    This contract is for providing meal services to patients and staff at the St. Wojciech Hospital in Gdańsk, located at Al. Jana Pawła II 50 and ul. Wejhera 12A. The contract consists of one part, with detailed descriptions, assortments, and quantities outlined in Annex 1A of the Terms of Reference (SWZ). The execution of the contract is defined in the draft agreement found in Annex 3 of the SWZ. The quantities of meals indicated in Annex 1B (Price Form) and Annex 1A (OPZ) are estimated and maximum amounts. The actual number of meals will depend on factors such as the number of admitted patients and available financial resources, and may be reduced to meet the actual needs of the contracting authority. The contracting authority requires that the contractor or subcontractor employs individuals performing tasks related to the contract under an employment relationship, as specified in Article 22 § 1 of the Labor Code. The contractor must provide a list of employees under employment contracts by the start date of service provision. This obligation extends to all subcontractors involved in the contract. The contracting authority has the right to verify compliance with these requirements. Upon request, the contractor must provide copies of employment contracts and current employee statements confirming the continuation of employment. The contractor must secure the salary information in the provided copies. Participation in the procurement process requires a bid bond of 30,000 PLN, which must be submitted before the deadline for offers and maintained until the offer binding period expires. The contracting authority does not require the submission of specific evidence. Eligible contractors must meet the participation conditions and not be excluded due to circumstances specified in various articles of the Public Procurement Law and related regulations. Before selecting the best offer, the contracting authority will request the highest-rated contractor to submit current evidence within a specified timeframe. The contractor is required to provide a performance bond of 2% of the total gross price stated in the offer. The contracting authority reserves the right to personally perform key tasks outlined in the contract, particularly those related to meal deliveries to distribution points in Gdańsk.
