    Brief

    This contract involves the supply of medical equipment for the Independent Public Healthcare Facility in Łuków. The detailed specifications are provided in Annex 2 of the contract documentation, which includes a list of technical and functional parameters. The procurement is divided into three packages: Package 1 includes a compact cardiomonitor/patient monitor, a modular cardiomonitor with equipment for the IT/Recovery Room, and a central monitoring system. Package 2 consists of an ultrasound machine. Package 3 includes an ECG machine with a trolley, a clinical defibrillator, Holter ECG, Holter blood pressure monitor, intubation set, infusion pump set, volumetric/nutritional pump, AMBU bag, and a sphygmomanometer with a cuff, as well as a treadmill and stethoscope. General requirements for the equipment include: 1) it must be brand new, produced no earlier than 2025, marked, and ready for operation upon installation without any additional purchases or investments; 2) it must be unused, not previously displayed or presented, unrefurbished, and free from physical and legal defects; 3) it must meet the relevant legal requirements for use in healthcare facilities; 4) it must have a medical device declaration and a CE certificate issued by a notified body, in accordance with the Medical Devices Act of April 7, 2022; 5) it must ensure the safety of patients and medical staff and provide the required level of medical services; 6) it must not be scheduled for withdrawal from production or sale by the manufacturer at the time of the offer submission. The contractor is not required to personally perform key tasks. The contractor must secure their offer with a bid bond of: Package 1 - 3,000 PLN; Package 2 - 1,500 PLN; Package 3 - 5,000 PLN. The bid bond must be submitted before the deadline for offers and can be provided in one or more of the following forms: 1) cash; 2) bank guarantees; 3) insurance guarantees; 4) guarantees from entities as specified in Article 6b, paragraph 5, point 2 of the Act of November 9, 2000, on the establishment of the Polish Agency for Enterprise Development. The required delivery time for the order is 42 days from the conclusion of the contract.
