This service is crucial for improving hospital discharge flow, reducing hospital readmissions, and delivering better health outcomes for Herefordshire residents. The contract runs for 12 months with the option to extend for up to an additional 12 months. This procurement is being conducted under the Light Touch Regime (LTR) under the Procurement Act 2023, Section 9.
