    Brief

Herefordshire Council is seeking an experienced provider to deliver a Discharge to Assess (D2A) Reablement Service supporting adults transitioning from hospital to home-based care. The service will provide short-term, therapeutic reablement interventions lasting 2-4 weeks, focusing on restoring functional ability, promoting independence, and building confidence in daily living activities. The successful provider will adopt a person-centred, therapy-led approach and work collaboratively with Herefordshire Council, Wye Valley NHS Trust, and the Integrated Care Board as part of the integrated home-first system. This service is crucial for improving hospital discharge flow, reducing hospital readmissions, and delivering better health outcomes for Herefordshire residents. The contract runs for 12 months with the option to extend for up to an additional 12 months. This procurement is being conducted under the Light Touch Regime (LTR) under the Procurement Act 2023, Section 9.
